Files All Participants in Upper and Middle School athletics MUST have a Physicians Certification and Parental Consent Form turned into the athletic department prior to their participating in practices or games. The form is valid for 365 days after the exam date written on the form. After that the student’s form is expired and they will not be allowed to practice or play in games until a new form is completed.
Upper School students who are interested in receiving credit for an Independent Athletic must fill out this form completely and turned into the Athletic Department or the Registrars Office prior to the beginning of sports season for which you are trying to receive credit. Applications will be reviewed by the Athletic Department. If you have questions, please contact Guy Garcia, the Director of Upper School Athletics, at 303-914-2574.

Only use this form if ALL INFORMATION is the same as used in a previous year. Applications will be reviewed by the Athletic Department. Middle School students who are interested in receiving credit for an Independent Athletic must fill out this form completely and turned into the Athletic Department prior to the beginning of sports season for which you are trying to receive credit. The Athletes and Parents Handbook contains important information regarding the CA athletic program. Each student/athlete and his/her parent should be familiar with the information contained in it. At the end of the handbook can be found the Athlete's Code of Ethics that must be signed by both the student/athlete and his/her parent in order for the student/athlete to be eligible to practice and play. Please download page 15, sign it, and return it to the athletic department prior to the start of practice. Thank you.
